Patient is entitled to the Medical Services identified below, as deemed appropriate under the circumstances, at the sole discretion of Your physician. Unless otherwise stated, the Patient is responsible for all costs associated with any medications, laboratory testing and specimen analysis related to these Services. • Acute and Non-Acute Office Visits • Preventive Care • Annual wellness visits • Sports Physicals • School Physicals • Well-Woman Care • Well-Child Care • Newborn Care • Electrocardiogram (EKG) • Blood Pressure Monitoring • Chronic disease management (e.g. diabetes, high blood pressure, asthma) • Glucose Monitoring • Breathing Treatments (nebulizer or inhaler with spacer) • Cholesterol Panel • Urinalysis • Rapid Strep Test • Suture Removal • Ear Wax Removal • Administration of Vaccines (Patient Responsible for Cost of Vaccines) Non-Medical Personalized Services. Members are also entitled to the following non-medical services and amenities: · After Hours Access. Subject to the limitations of paragraph 15 of the Agreement, Members will have direct telephone access to their physician for emergent concerns which unexpectedly arise after office hours. In addition, video chat and text messaging is available when the physician and Patient agree that it is appropriate. · Direct Electronic Access. Members will be provided with one or more means of electronic access such as email, patient portal, text messaging, etc., through which they can directly contact their physician in regard to nonurgent matters. Such communications shall be dealt with during regular business hours. The Member understands that email, text messaging, and other forms of electronic communication should never be used to obtain medical assistance or care in an urgent situation. Accordingly, the Member agrees that in any situation which could reasonably be expected to develop into an emergency, the Member shall immediately call 911 or proceed to the nearest emergency medical care provider. · On-Time Appointments. Subject to the limitations of paragraph 15 of the Agreement, Members shall be seen on time for scheduled appointments. If Your physician foresees more than a minimal wait time, You shall be contacted, advised of the delay, and given the option of rescheduling Your appointment. · Same Day/Next Day Appointments. Subject to the limitations of paragraph 15 of the Agreement, Patients requesting it shall be scheduled for same day/next day appointments as medically necessary and appropriate. · Specialist Coordination. Your physician shall coordinate Your medical care with Your outside medical specialists as appropriate. This Agreement does not cover or include specialists' fees or the services of any medical professional other than the Practice staff.
